I had it a couple of times while raiding, so I don't think it only comes up when someone else is hitting as well.
Might be a secondary message for realm alert instead of 'blah blah, 6 seconds until...'.

The message H.E got it when 2 people are hitting the same person, i got it alot last night when massing, i think more because with so many on PPT and that game crash, that as soon as someone came off PPT everyone wanted a piece of them lol.
