Dare I suggest...the U.S. Federal government? For stating, quite clearly, in the constitution...any state may withdraw from the union at any time...but stating that formation of the Confederacy was rebellion?
(single bloodiest war the U.S. has ever fought, bar none...even WWI and 2 don't beat it)

not really a person but religion topels the kill count 
For persons tho, Hitler would most likely be the no.1 dunno bout many of the others.

I believe it depends if you count the millions who died due to his reforms. Such as from starvation after the Great Leap Forward, etc.
