It's the same like in SGW: some play on the forums (the big teams who think they are winners before the match even started) and some play on the battlefield (the working teams - those who farm naq and speak less - those like Slovenia, Paraguay, Uruguay, etc).

If England beat Slovenia and USA beat Algeria then England go through.
If England beat Slovenia and Algeria beat USA then England go through.
If England beat Slovenia and USA and Algeria draw then England go through.

The only way England really go out is if they lose to Slovenia or if they draw.

But still, another piss poor performance.

Most likely in the three you listed.
I mean, heck, Slovenia led by 2-0 and USA obtained a draw in the end and could have won if referee didn't hallucinate.

Its like watching Liverpool from last season watching England....The players are good enough but they don't do squat and look at each other like it's someone else's fault....Rooney instead of looking pissed off when you lost the ball run after the guy and win it back...
