General, God, Fleet Commander...how do you get them?

Ok, i'm not so much a newbie, but I have to ask this because I can't work it out.
You have God, Fleet Commander, General Commander etc.., but where do these spur from...it never works as it is meant to!

These are commander titles, and there are 'top' officer ranks as well (First Prime, Colonel, Second etc.). You get these titles by getting officers/being your commanders best officer.
Also, if you can amass enough officers, you get an even more sparkly title.

Also the favored officers of the ascended get new names as well, such as right hand or prodigy (dont know the other two)
