how do i get a sub tittle

so i was woundering how do u get the sub-titles that some players have...(not the title you get with ascending). here are some users with them:

if the others are old enough would that not make that one old enough as its the lowest one

I thought i saw 6 numbers but there are 5 my bad to 10208
