Ingame - Never, although I did once mess up my 6 monthly password change and admin Sue god bless her sorted it for me very quickly
Forum - Once, Deni provoked me and I was quite rude to her, I was angry at the time but in hindsight we were both acting very childishly and have since both apologised ( for that incident if not others

Never ingame, twice on forums, one was by psi back in 2006 (wasn't even for reaching 3 warnings lol..I had 4 warnings in five years, less than one warning a year average), and the second time, I requested it (cool down ban extended).
